PHP MYSQL Project Cleanup

This project was started over 1 year ago, and was handled by 2 developers in India. This project is quite complex, and unfortunately has not been handled correctly to date. Fortunately, a Canadian development firm has taken control over it but in order to get this project done on time we need reliable support. We require a developer that can work quickly, efficiently, and that will not make silly mistakes. Your English language skills must be perfect, both verbal and written. You will have to chat over Skype as part of the selection process.
The project has been designed and architected. It consists of 10 PHP pages, with each one containing 6 survey style questions. 2 of these 10 pages are complete. You must simply follow methods/procedures that have been previously used, and finish the remaining pieces of work.
Here is a summary:
The questions asked on these forms range from text boxes, dropdowns, to radios, to check marks. The user inputs their answer and it records to the database. All answer tables, as well as tables for question data (i.e. options for dropdowns) have been created.
The original devs hardcoded all the answers to the questions. For example, if a dropdown asked you to select your favourite colour, rather then having a database table called faveColours where options could be updated/corrected/etc., the options are hardcoded in HTML. Tables have been created for answers, and now you must write the queries to call them properly for each type of question - i.e. calling options for dropdowns, radios etc.
Be warned, that one of the most complex aspects of this form is that many require an 'other" answer. This means that if the user does not like one of the options, they can select something that will generate a textfield for custom input, which must be recorded to the database in the same way that the "normal answers' would be. This has been a challenge for all developers to date.
Additionally, you will have to do some style and design work - this will be quite basic though. For example, some dropdowns may not be aligned to their text, or some text fields may not be the correct height. I don't expect world class graphic design, just something that is aligned and clean.
There may be minor tasks within this that require ajax/jquery knowledge. Of course, you will need to know HTML5, PHP and MYSQL. This module does load within a Wordpress environment, but is stand alone - i.e. we could rip it out of wordpress at any moment and it would continue to function.
As I have already had serious setbacks, this project would work with short, aggressive milestones. The contract value listed here is for all remaining pages. You would be expected to complete this job within approximately 7 days. My Canadian devs will review your work, and if it is not satisfactory the contract will be immediately cancelled, and the next candidate will be contacted.
In short, you must be able to follow direction, the work of others, work quickly (with no excuses or delays) and provide error free work. If you are successful, I am confident that future projects will be yours to be had.
