Secure PHP Contact Form

I need a PHP contact form to replace my current solution at [login to view URL] This one is getting spammed too hard. However, I need it to have a certain number of features. - Captcha and turning question security vertification. Captcha image should be relatively simple. Which type of security to use should be selectable in the code. Only one is nessicary per form. - Ability to ban ips. - Autoresponse to user - Mark some fields as mandatoy. - Vertify email field is valid format Script should be contained within one block of code that can be inserted to a page. Call to external file can be made for script fuctions if it will improve page loading times, but all variables such as feilds, autoresponse, emails, etc muct be contained within the main page. Banned ips should also be set from a signle file. Reason for this is the contact form will be used multiple times on the same website for differnt purposes with differnt recipitants. So basically, someone can fill out the form like the one on the page above. However they will have to imput the secuirty code to vertify they aren't a bot. The script will also check to see if the email is in valid format and that all mandatory fields have been filled in. The user should be promted to fix the errors if there are any. The contents of the fields will then be sent to the email set for the form owner is a easy to read format. Date, Time and IP should also be included in this email. An autoresponse will be sent to the email of the person who filled out the form. The text must be set in the code and can draw on form fields, ex: Dear *Name*. Lastly, thery will be redirected to a set page upon sucessful completion of the form. There are many open source examples of similar scripts to this on hotscripts and other sites. I don't mind you using open source code as long as modifications are made so it matches the above requirements.
