Hi. I have an existing site, with a page holding an Address Book (bootstrap dynamic table). This table has a button "Add New". I want someone to write code for: - Open a bootstrap modal on clicking the "Add New: button; - Create a form inside this modal, for a user to fill 6 inputs; - Add a "Submit" button that POSTs these inputs to the original page. The Address Book page ([login to view URL]) is already set up with the table, "Add New" button, and code to INSERT the posted inputs to my database. I am building this website myself, but due to my lack of developer knowledge, there will be plenty of similar tasks to be done, so further work is likely for the right person. I have attached a basic brief with some pictures. I can send the existing code (with bootstrap, js, css etc.) once job is awarded. Thanks, Nigel
