6-page template website design job - non-flash

I need a designer to find and purchase a template to build a website similar to [login to view URL] Designer is required to locate this (or closest resemblance) template for me to purchase. My website is for a clinic targeted at the whole spectrum of beginner to advanced athletes, offering services such as coaching, sports monitoring and performance enhancement and health/wellness. The main purposes of the website are to: - introduce the visitor to this new concept of a health/wellness clinic specially for athletes - get sign ups for free membership (by offering free e-book and other freebies) - get visitor to make appointment Attached is the site map as well as my suggested 6 templates that I will need with the design elements in place. Website functionality: - no flash, please use jquery - will provide some graphics, may need designer to locate appropriate stock pics to purchase - mainly a static website, will need content management system for self-update - no database, no membership module needed - will require 2 simple forms, for enquiry and making of appointment I prefer that the designer purchases a template instead of building from scratch due to cost concerns. Awarded designer should find the templates for my approval. I will purchase the template and hand over files for work to be done. Timeline: 2 weeks maximum
