Adapt SuiteCRM to specific task

I need help setting up SuiteCRM Version 7.6.4 (Sugar Version 6.5.23) to do basic routine work as follows: - the system is already installed and running, but users and permissions need to be adjusted. - The main task to work on for this project is setting up a form for users to fill out. It will be a custom form with text and dropdown menu input. It is basically a logging system, with a possibility for the manager to view a report that combines input data by other users (employees). Users should be able to login with their own login info. - I assume most of the work will be done in the studio section. I expect you to be familiar with studio. - i need someone to be able to do this through teamviewer and skype chat. please disregard this project if you not are able to. Otherwise, please provide me with your skipe username to discuss the project in details. - This project is about setting up the architecture and doing the basic setup.
