CREATE MASTER DETAIL SCREEN IN FRAMEWORK YII2

I am a desktop developer learning migrating to web I m developing a system for my church in Brazil. To do this, i am using framework Yii2 by friend recommendation I need someone to develop for me a screen style master-> detail (1) e detail(2) I have the tables schemas.. i have the application almost ready just missing that. I ve attached some files to make easy the understood. The objective of screen is register the home meetings - date time, and the members and visitors. Master - > Lanc Celula Detail 1-> Membro_lanc_celula Detail 2-> Visitante_lanc_celula I will send ftp access to the project. The lay out is attached aprox is here in the project. The member will come from members table using a windows search The visitors can be type directly dont need search form Any doubt please post here.. Time: 5 days max.
