Create a Wordpress Plug In

I need a WP plug in created for my website.

I would like to be able to add my own images on the first selection for Male or Female. You'll see the Male/Female option that you can click.
1. Ask a question (like Male or Female?)
2. once clicked (Male/Female), new questions are presented based on what they chose.
3. Once they choose another option, then text will appear and a video embedded from Youtube for them to watch.
There will be a place asking if they would like to Download something and schedule a class. Once they enter in there email and pick a class day the page will redirect them to our standard thank you page.
I need it to loo good on mobile phones which means it will have to switch from left to right to Top to Bottom so the text is large enough to see

I will chose my top favorite 4 freelancers. I will pay you to work for ONE full hour to get as much done as you can. At the end of the ONE hour you will present the quality of your work and a small demonstration. Who ever has gotten the farthest with the best work will get the whole project. If you do not win you will still be paid for ONE hour only.
