Create a data input program in excel

I am in need of an app that will allow me to client information from a user interface into an excel spreadsheet. I will need client info entered(12 fields), as well as an on site interface were the client is selected and 7 more data points(numbers) are entered and stored. I will then need to be able to recall the data in a preset format for printing. The data entered as numbers will need to be manipulated with preset formulas that do not change, such that it can be printed out. I will also need to be able to print out 3 different printouts.
I need to add more info- Ideally I would love to have this operate on my phone, or on a small laptop such that I can go on site, collect and enter the information, and print out the receipt to the client. I would then go home and sync the info to my business computer. Thanks
