Create a simple web based customer refereal Portal

Front End (very basic): 1. User logins in to organisation account using their username and password (that is assigned and created by an admin). Then should then see a form, which contains the following fields: 1. The ability to choose who they are within the organisation (a drop down box). 2. User then enters the following details: a. Customer Name: b. Customer Phone Number: c. Customer Email: d. Comments: Customer Name and Phone Number are compulsory fields and must be field out. Also, able to easily add more fields latter on if necessary (similar to contactform7) Once they have filled in the form, there is button to submit it. Once the form has been submitted, all the data needs to be saved in a database for use in the backend. Form Submission Conditions: If the Customer details (Name, Phone Number or Email) are already in the database, then an error message should appear: “Sorry that customer is already in our database.” If the form is submitted successfully, then a message should appear: “The customer’s details have been successfully submitted.” An email with all the customers’ details would be sent to the specified email address/addresses in the Admin Panel. Back End: The back end needs to have an Admin area/panel which allows: 2. The ability to add an organisation Account (Name, Email, Phone, Address). a. The ability to add People within each organisation (so they can be selected from the drop down box in the front end). 3. Ability to search and browse database: a. Browse by organization, persons within the organisation. 4. Change the email address the form is sent to: a. Able to make changes to the email body and subject of the email based on variables pulled from the form and database [organisation-name], [customer-name] etc. (similar to contactform7) b. Ability to send the message to more than one email address. 5. The ability to export the database for each organisation as a CSV. Installation: Need to have an installation php. I just set the table name and password in an [login to view URL] file and then run the [login to view URL] which would create the appropriate tables. Then I would delete or rename [login to view URL] (i.e just like a wordpress install). Forget Password Option: A forget/reset password option for both admin and users.
