audio perception quiz

I do research on human perception. I want to make an interface to let users play sounds and answer a question (the type of sound) (radio buttons), and use slider bar to select the intensity level, for instance, and submit button to submit their answers and go for the next sound. There are 9 sounds which repeat randomly 5 times. Every time they submit their answers they will receive the correct answer in a popup window, or check mark next to correct radio button, etc. Also, once they click on the play button to listen to sound, and once they submit, the time they spent needs to be recorded as one of the data in addition to their radio button, and slider level answers, in a database. At the end, once all sounds repeated 5 times randomly (which is 9 sound*5 repeat=45), there is a test, they take. The test, uses the same format and sounds, but 3 times randomly repeated, and there is no any correct answer feedback. I have developed some parts, but as I am approaching a deadline, I need some help.
