Build me a program that can propagate pre-made forms based on someone's input.

I'm an attorney who needs to fill out certain forms on a regular basis for every new client. The forms are from the United States Patent and Trademark Office (USPTO). Basically, I want my client to type in a form that YOU will make, and the information they type in will automatically go into the form in the link above, and also a few more easier forms. Relevant information includes: - Person's name and address -Company's name and address -Small entity checkbox -Among others. The Form above is the most difficult. All the other forms that i need automatically filled out just includes the name and title for people. Please let me know if this is possible and what the budget should be. Thanks, -Tom
