Build a simple website to validate a document code

Hello all. We need a mini site with our company logo, an input field and a "check" button. Basically the user must insert a code and the system must answer if the code is valid or not, the expire date of the document and the subject of the document. You must supply also a valid tool to easly connect to the DB and add more document data to the db. We will supply an EC2 small machine, and the freelancer should take care also of the configuration. The website should be accessible from Internet and the "check" button shall be injection proof.
