Hello Marco, I can do what you are looking for using ruby language and some external tools (all of them are free & open source software). My solution will be a cross-platform software that can run on Windows/Linux/Mac OS X. It'll be done as a command line tool that takes the required directory with your PDF files and creates the resulting PDF file as described.

Hi Marco. Probably I’m the person you need for your project. As a matter of fact for more than two years I worked as a leading software engineer on an e-signature system called SignCenter. Its target market was US healthcare segment. The system backend was implemented on .NET. Input document format is either PDF or multiframe TIFF, output format is PDF. It even allows to import interactive PDF documents and convert PDF Form fields to internal layout format. I guess it’s obviously that we also had to write tons of nontrivial code related to PDF processing. To get the job done we were mostly using PDFSharp and AbcPdf. Even some low level manual PDF “gluing” was done when it was necessary to achieve better performance. BTW. Do source and target documents in your case have the same size (so that source documents should be scaled down) or target page size allows to fit four source pages without scaling?
