Make a review system to price comparison wordpress site

Hello I have made a wireframe, so people can have a look on what i want. (You can change sites up in the left corner) [login to view URL] (please look at the comments on the site) So this is basicly what i want. My website is: [login to view URL] theme: [login to view URL] List of what i want: 1 - I want a buttom leading to the form on the frontpage in the header. 2 - I want the form to be FULLY functionable. 3 - Backend and frontend coding. Notes. If you didn't read the comments on the wire frame. You HAVE to choose a "Leverandor" (supplier) to leave a review on. Its optionable to choose a Product, and what product it should be. (Product should be a dropdown menu with all the products listed in the store). Choose and searchable. Same thing with Supplier, but you MUST choose a supplier (leverandor) to procceed. You also need to be able to upload a picture, to proof of the review. Every field is nesseseary to fill inn before you can send in the form to "review". We need some sort off accepting/ declining the reviews sent in. If someone leaves a review on a product with a supplier (leverandor). It have to appear under the product review, with store, comment, proof. If someone leaves a review on a supplier only, it appears under all leverandors reviews. (not created yet) Info buttom down to the left to show the info site. Like a lightbox.
