Need a HTML champ -- 2

Hi there, I am attaching one image. I will provide PSD to the awarded bid. I need you to make it in HTML and 'editable HTML' so that I can edit the CV and there will be a 'Make PDF' button which will make it downloadable PDF immediately after it's edited. So this image will be shown at the site and people will edit it and click 'Download PDF' button to get the PDF. In the image, all are text field except at the bottom left there is star field and on bottom right there is a drag module. For PDF you can use many online available engines, tell me a way how you will convert it to the PDF in the bid and I will award you. I have 15$ and I have one day. No milestones will be created unless I am sure myself that you have done the work. So you can upload in your server so that I can't cheat you. PS: You can't write this in any portfolio of yours. The final page will look like [login to view URL] , in the left side, there will be the editable CV and the right side, one button 'Download CV'
