Order Form for 2Checkout

I need to have the following order form [url removed, login to view] created.

The form must do the following:

1. Once the form is filled out all the information should be transferred to the 2checkout payment screen.

2. Form must also send a confirmation email to the customer and an email to the admin email account.

3. Form must update the customer database to add this entry into our database only if the transaction is approved.

4. Form will need a config file where the cost of the product, description and 2checkout id can be stored.

Please do not bid if you cannot work on this project today because I need to have this done by the end of the day.
