Perfex CRM Module Build (Codeigniter)

Hi, I'm looking for somebody to create a module that works with Perfex CRM that will allow me to create a questionnaire-based form, with picture attachment which will be accessed by the admin, once they've submitted the document this will be accessible by the respectful client only and therefore should contain an obfuscated link with an option to force the client to login, this setting should be a system based config. The output document should be listed on a page similar to the views within Perfex already uses but, the module is to ultimately generate a report and than be emailed as a PDF with a logo and to pull in the customer's contact details and address. There should be headings and can be alerted (as a template) with picture attachment. This needs to all be combined as a PDF. This is to create a building surveyor report that can be exported as a PDF or emailed as HTML based report.
