I have a form that is used to RSVP to an event. Users enter in their name and address and that is written to the database.

I need to update so that each visitor will be able to click a checkbox to that will allow 2 additional names and addresses to be entered (so they have the option to bring a couple of friends to the event) These names will be attached to the id of the person who is filling out the form (they are guests of the invitee)
