I use a contact form PHP script that I found somewhere on the Internet, and I want to add some additional form fields, such as corporate section, job title, address (physical), and phone number, on the existing PHP contact form. This form consists of three pages, [login to view URL], [login to view URL], [login to view URL], where [login to view URL] is the input page, [login to view URL] is the confirmation page where the user can either submit it or go back to the input page to amend the input, and [login to view URL] is the result page showing if submission is successful or not.
