Simple Website Makeover

I need a few updates to my new site/blog. [login to view URL] Underneath the banner I want a banner ad that links to the free analysis page below. I need a few static pages: Services Free Analysis Contact I also need a website analysis software installed. [login to view URL] There are two files...one is the original...and second is an update because it apparently wasn't working on some databases. I also need the free analysis page to have a form created like this one: [login to view URL] I want the Sponsors deleted and the image below it. I also want a logo that I had created installed in the header (I have the psd files). [login to view URL] In addition where the 336x280 google adsense image is below the Welcome area on the upper right where it says 'Not A Green Thumb?'
