Microsoft Forms - Troubleshoot error

I have form created in Microsoft Forms to capture some user inputs. I am getting this error as attached. (This form does not exist error). I have checked link and link is fine. In addition, the error comes sometimes only. You will need to connect to my system remotely and try to troubleshoot and provide solution.
The form is located on SharePoint site and data going to an excel sheet
