Design website accepting forms from users (10-15 pages)

Greetings, We're seeking an experienced designer to design our new website for us. The site will consist of approx. 10 to 15 pages: 1. Home Page (very simple, with an intro video, one CTA button (Submit Form), and all the basics) 2. Submit Form page (user will enter a fair amount of information in this form) 3. View/Review/Edit submitted data page. 4. Invited Friends page (show list of invited friends, monitor if they visited us, provide ability to invite friends via email, and a manually drafted message, etc). 5. F.A.Q. 6. About Us 7. Terms of use/Policies Admin pages: 8. Filter page (filter submitted data) 9. Filter results page Client Pages: 10. Filter page (filter submitted data): A more attractive version of admin version 11. Filter results page (A more attractive version of admin version) Although similar, the functionality will vary between the Admin, and Client pages. I am certain there will probably be a few more pages- hence my estimate being in the range of 10-15. You can see, most of these are relatively simple/straight forward, once the style, header/footer, of the home page will be created. We're not looking for generic, 'safe', or standard designs. We're looking for designers who take risks, and break the mold. Please include samples of some of your coolest/most unique designs. I understand, the most annoying feature of Freelancer, is the automation of submissions. As soon I hit Submit on this job, I'll be bombarded with 50 offers within 10 seconds (enough time to read the title of this job posting, nothing more). I created one myself, and - feedback is positive, but, I think it might need a subtle touchup from someone with an eye for branding/designs, etc. Finally, we will need both Web responsive design, and Mobile responsive design here. The final designs will be presented to us in psd files, or whatever the preferred standard is by developers these days (I believe, psd's are the defacto go-to format?) In terms of timeline, I'm generally very flexible. However, in this rare case, we need to get started on this project with utmost urgency. Thus, we will mostly look at those who can deliver this very quickly. To make your life easier, I'm very responsive, and work online all day. I'll be able to provide you feedback in real-time (during working hours and evenings, European time). Please provide a bid that is binding. Few things annoy me more here than 'placeholder' bids.
