We require someone to create a logical form to be used on our clients site, users will select a category from a drop down, then depending on that choice a second set of categories will be displayed, when they choose from that a form that we have already created will need to be displayed (we have shortcodes for the form to insert). Forms we are inserting are created using ez Form Calculator and we have FormCraft installed. Final form will have to be coded to open a modal popup.
