Contest Brief

We have a competition website, when someone wins we want to send the prize along with what we call a "Winners Card" which will be A5 in size.

I want it to have our logo which is attached and writing saying the following -

1. CompetitionGo (at top)
2. WINNER (winner)
3. WWW.COMPETITIONGO.CO.UK (bottom
4. Fields required: Winners NAME / DATE OF WIN and a place for us (CompetitionGO) to sign.

Simple and clean but the main thing is having a fun background see our website for current images/theme we have I have also attached an image of the theme we use, don't use this image but create somthing similar.
