Contest Brief

We need a quick html page for one web page. The elements needed are illustrated in the attached pic.
The main color of our website is the green color of the button on the pic.

In the middle of the page, the user may select Bid Type: per click, per thousand views, or per day. A dropdown selector should be used. The user will select one of them. Then in the amount row, the corresponding clicks/thousand views/days is displayed.
