What i need is something that will have the following fields, Also all of the fields are madatory.
- Name
- Email address
- Confirm Email address (make the two email fields check each other that they are the same)
- Fiield to enter some text (will be a list of names)
- A text box with scrolling text (this willl have the Terms and Conditions
- A check box that must be checked inorder to proceed
- A submit button
Then once the data is entered this needs to be sent to an email address and the email address and name put into a database. IF you have any questions about this one let me know or if there is anything you would need from me let me know.
