Advanced application form that generates organized PDF files with a customized design. This form will need to have a photo of an applicant, about 20 text fields, and capable of uploading three additional files. The form needs to run on smart phones, and needs to be secure. Additionally, submitted forms need to be stored in a database and found easily.
