Create two simple ms word templates

I have two simple templates that need to be created. I will scan and send the two as an attachment upon bid approval.

Template 1 has a fixed return address, the word memo and three lines of input, from; date and subject;

Template 2 has a fixed title, footer and three fields, Prepared for, type of coverage and date.

There should be more template work after completion, no guarantee but I expect there will be more.
