Blockchain Proof-of-Concept: Supply Chain Management

I need to build a Blockchain demo or proo-of-concept (PoC) using a Blockchain of your choice (ex. Hyperledger Sawtooth). The PoC should show how Supply Chain can be addressed by utilizing Blockchain such as a secure audit trace of what happens to business assets and processes. In addition, it must powered by Blockchain and should provide an API that is easy to integrate into legacy systems. The demo should have a simple UX/UI that can be accessed via the web interface (here's an example of what I'm looking for [login to view URL]). Just as TracyChain (see previous URL) the concept should show when an item is created, a corresponding digital token is then issued by that entity, which will act to authenticate its point of origin. Then, every time the physical item changes ownership, the digital token is moved in parallel, so that the real-world chain of custody is precisely mirrored by a chain of transactions on the Blockchain. The token is acting as a virtual “certificate of authenticity." Upon completing the project, I will need full access to all of the code, for experienced Developers this shouldn't take too long to create. Lastly, I'd like to request that the PoC to be coded in the following code-base - Java, Javascript, Python or C# (in this list of priority).
