JQuery Questionnaire Scripts for Admin/User sides.

I need Questionnaire Expert.

I've already a asp.net MVC Questionnaire web site. I need some kind of 'question types' to ask a user.

There is several kind of question. You can find the example images on attaches.

Each Question has on one and only page. There is no several question on one page.

Use clean JQuery code. Make sure it will work most browsers. Do not use obsolotes.

I am using some text line for the choices but If we talk about multiple choice I could use some images for each choice like following...

I Prefer Question and Choices are RichText area.

I will use Razor engine. Please supply me razor code for the admin and user side.
