Expert [login to view URL] Database LMS Developer - debug mdl_grade_grades offender

Our company uses [login to view URL] to deliver content to end users.
Over the past ~2 months there has been a specific query (mdl_grade_grades) that puts our database's CPU ~100% and results in 504s delivered to students.
We are using AWS RDS for the database, we were using [login to view URL] but after this offender became a regular occurrence we upgraded to [login to view URL] RDS instance.
Since the RDS upgrade the offender has "almost disappeared" however we have had 2 specific instances where the new RDS CPU reached 90% usage and 504s where delivered to end users for this same table/query. As user activity has increased so has the frequency of the offender.
We are in need of a moodle expert to help us figure out the root cause of this offending query.
