I need code for 2 web pages. (1) A web page with a simple form that e-mails results to a recipient and stores form results to one table in a local SQL Server database and (2) a page to view form results. No graphics or design needed, I'm plugging code into a template. All must be done in .NET to run on 2000.
**First page** will include a simple form (form provided - see attached). The form results need to be e-mailed to recipients from form, and sent to a database on the same server (currently using ASPEmail; configuration attached). The database consists of one simple table, solely used for form results.
**Second page** will display a summary report of the form results stored in a SQL Server database. Results will be shown as, "Orders per day, week, month and year-to-date". Timeframes should be derived from date stamp on form.
**Note:** Please include explicit instructions within code on where to insert database connection and path information
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request.
3) Exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
