PDF Document concatenate merge in PHP

I am running a very small business where members sign up online, then we create an application form dynamically which we get them to sign (physically) as a contract.

There are a few parts to the documentation that the member must physically sign;

1. Dynamically created application form (in php/html)

2. Standard form contract (already in PDF) - no customisation

3. Standard form membership manual (already in PDF - no customisation)

4. Maps of areas to which the member has access (dynamically created - sourced from mysql database and image files uploaded to a web page)

I want to be able to create one pdf file, with all 4 elements in it, which my administrators can download, print, and present to the member.

I would like to be able to change the contract/member manual by uploading a new one to the website if and when our contractual terms change.

You will;

1. Code me a script which will combine the 4 elements into 1 pdf file (i already have FPDF installed, which you can use)

I Will;

1. Provide you with the existing code that you can use to create the application form, the standard pdf documents which must be combined, and the information you need to generate the map files.

Constraints;

1. App must be in PHP

2. App must be able to run on remote server

3. App must not require any funky libraries.

PLEASE NOTE: If pdf is the wrong format for this (ie you think you can do it in another, printer friendly format) then LET ME KNOW, And MAKE A SUGGESTION.

I chose PDF because its a pretty friendly format, although all the actual pdf docs were created in word (haha).

Ultimately I would like to be able to email the combined document to the member (instead of me printing if off), so thats another reason I chose PDF.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).

## Platform

PHP on remote server, to be served up as a webpage where i can download from.
