Simple Dynamic Contact form and Facebook/Twitter Share Abilities

A simple contact form with Upload picture/Doc file abilities, and the successful page to be dynamic towards one field from the contact form.
For Example, John Doe fills out a form and the success page to say, THank you John Doe for filling out the form.
Additionally, Once the success page is loaded the user is able to share his submission confirmation on Facebook and Twitter.
The Facebook share will be a Preset Picture and a statement like "Joe Doe Submitted This Form"...
The twitter just needs to Tweet the same thing along with a Preset Hashtag and Preset Tweet Mention So something like:
@CompanyName #SubmittedForm
