Tax calculation web

I need a simple web with calculator and database connected to it. Explanation and formulas are in file attached.
Croatian cities and streets should be in the database and recognized as typed in text box(also quote how many hours of work without this option)
Visually page needs to be very simple, maybe light blue with big smooth white text fields and light blue buttons with white edges.
Left should be floating social plugin and on the bottom, as option, possibility to send calculated values from table to personal email.
Please send estimate of hours needed to finish it.
Also, include what would be the cost of adding a possibility of embedding the calculator on other pages(like embed code on YouTube)
