Fill Adobe Form From FDF

Need a script in PHP that will fill a PDF form that is already designed. All the fields on the form are known. The values of each field will be generated in PHP. Some of the fields are text and some are check boxes. I can fill the form now by generating a FDF file, then clicking on the FDF file to load the values on to the PDF. This is not desirable. The values from the FDF file must pass automatically to the PDF file without the user intervention. PDFtk or any other tool kit or third party application must not be used in the design. Only Javascript or straight PHP should be used. Please see php script below which is now being used , but needs user intervention to fill PDF electronically from FDF
