PHP Custom Script - Check for Extra and Missing Files

We need a php script/page that can run on a windows server (IIS not apache) and do the following:
Input for:
1. Microsoft SQL Server host/ip
2. SQL username
3. SQL password
4. SQL query to return filenames. Assume the query will always have only 1 field returned called "filename".
5. Source Path
6. Move Path
The script will read in all filenames in the source path directory. It will then compare each file in the path with the "filename" column as specified in the SQL query input by the user (1/2/3/4) and insure that the file exists in the dataset. If it does not exist, the file is moved to the move path (7). Finally, the table is re-read to check for the existence of each file in the source path. If the file exists in the query but not in the file system, write out to the screen the value from the column.
The solution must be provided as source code that is not obfuscated. It can be 1 or more PHP files. If you require specific libraries other than standard PHP libraries, please advise of this in your proposal.
