PHP MySql dynamicly add inputfield and store result in db page

For a website where students can order stuff needs these solutions :
Page 1: (student page)
create a form
get all rows from db table1 dynamicly (product_id, productname)
add one inputfield for each row
inputvalues must be sanitized and can be (decimal)numbers only
save all rows that are not null in db table2 (product_id, product_name, inputfield_value)
page 2: (a Staff page)
counting all products and inputfieldvalues and display like
product1 : (count sum all inputfieldfvalue for product1)
product7 : (count sum all inputfieldfvalue for product7)
product136 : (count sum all inputfieldfvalue for product136)
