Competition PDF poster and entry forms

I need an A4 sized poster for a retail competition made up.
I also need small pad, about 1/8th the size of A4, that we put in customer's shopping bags, and it reminds them how to enter the competition, which is online.
The competition poster will comprise of the following:
Headline eg. Win a sunny getaway for two to the Gold Coast!
Sub-headline 1 eg. Flights and two nights 5 star? accomodation!

Image eg picture of palm trees, beach, etc (you are to provide picture!)
In small text terms and conditions will be provided.
In the smaller form, I just need exactly the same. But the website address should be most prominent, reminding customer where to go.
