Form Calculator with modal pop up

We require a form calculator to be built the same as the cost calculator on Clockwise - [login to view URL] - , where the user selects the number of desks and years they would like to rent a desk for and a popup modal appears with the data.
We have a spreadsheet with formulas for 1,2 and 3 years and 2-20 desks with variations very similar to Clockwise which need to be reflected in the form calculator. We only have 1 location currently so the only variables are the number of years and number of desks. No styling is required. The end product will be used on a Wordpress website built in Elementor.
We require this to be done asap.
