Build me an app for iOS, Android and Web

I want to make an app to make registration of some procedures I do in the hospital. Could you help me?
First page would be the welcome and login page
There would be 4 types of users: Secretary, Admin, User, Nurse (by hospital)
Second page would be different for 3 types of user
User type “User and Admin"
A bar with the following options in the bottom: Users, Day, Register.
In the Users Section, there would be the following information:
Calendar
When click on a day:
Users name and position on the shift (first, second etc)
Hospital Name they are allocated on that day and the hour. Let’s say: 7:00 at hospital X and. 10:00 hospital Y
Day Section
Calendar
When click on a day:
All surgeries of that day, divided by Hospital
When the admin (only the admin) clicks on the Title of the hospital He can assign the users, himself included.
This new window could be a modal, with the following information:
-Title Hospital Name
-Name of the User (Choose from all users)
-Time
-Add a plus if he needs more Users
Surgeries listed data:
Start Time
End Time
Name of the patient, Age of the patient
Name of the surgery
Name of the Surgeon
Name of the Anesthesia
Name of the Anesthetist
Name of Health Insurance
When the User Clicks on a Surgery on the list he Is linked to the Register Section
Register Section
Form with the fields
Photo - with OCR that populates the following fields: Name, Registry number, Health Insurance
A field to register the code (based on two separate tables) - can have multiple selected
Name of the User (others can be selected, but the user that is logged in is the default).
Observation Field (Multi-line)
The secretary would make the input of data preferably through cvs file to the database in a web page or through the app (completing the form).
Data that the secretary can make input and change: Users name and position of shift by day and surgeries by hospital and day.
The Nurse user would have limited access to the app (Just to insert new surgeries and delete or change the data of the ones of the hospital that she is assigned). As soon as she adds or changes a data, the Admin user would get a Notification. Every change she makes has to be registered with date and time.
