.jsp/Java/MySQL web application for a public (State of New Mexico) Summer Food Service Program.
About 20 to 25 screens - need Design & Build based on solid Requirements document. No security/access needs, but complex form handling (Struts framework should work for most of it).
Need 4-Part "Sponsor Application" form debugged and delivered by 3/01/04.

## Deliverables
1) Complete and fully-functional working Java web application (.jsp on client side) in executable form, as well as complete source code of all work done.
2)Database design and documented schema
3) On line indexed and embedded reference HTML User Guide
4) Transfer, deployment and debug of developed software and database tables(in ready-to-run condition) to the hosting environment specified.
5) Exclusive and complete copyrights to all work purchased. (No GPL, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site).
## Platform
Application must be j2ee compliant, utilize MySQL or MS SQL database - needs to run from Unix/Linux & Apache/Tomcat container - no EJBs or Application Servers required by design, please.
