Building an order form for my small business, including colour, options and quantity, total etc I have attached a very rough copy of what I want below. I’m wanting it with an online file and also a printable file.
I have a simple form i need embedded into an email -- it only has 2 field, one being a Date Picker, and the other a dropdown list. I prepared the form in Microsoft Forms and cannot deploy this within my email - it simply attaches as a link. I read that this is possible via MS Office Sway, but this again simply pastes as an iframe code as a link and not does not embed the form in outlook. I added SurveyMonkey here as a skill but this is just one from so i do not want to be tied into their services and just want to email from outlook .

A google forms questionnaire shall be made available to a certain user group. We will provide the paragraphs. Every participant shall enter his postal address and then select (tick) paragraphs from the form that he can identify with. When done he shall submit and a PDF (Din A4) letter shall then be created containing the paragraphs he ticked. The PDF letter shall contain a predefind recipient address, a predefined introduction, a predefined conclusion and in between the paragraphs he ticked as well as his entered postal address for printing to be sent by postal service. All shall be done in google froms / google docs. There shall be no registration reqired for the user group. So every one receiving the questionnaire shall be free to answer it.
