Backstage Pass for School Event

I need a 2-sided 'VIP Backstage Pass' template for an upcoming elementary school event. The front needs to have a space for an approximately 2"x2" photo of each child as well as an area for the child's name and parent's name (which I can add one by one once the template is done). The front could look something like the attached file and perhaps the back could be a simple tie-dye image with the name of the school (which I'll provide later), the text "Backstage Pass", and the name of the event which is: "Ch-Chillin'". I would then print and laminate them myself. I am open to any ideas, but I don't have a large budget or a lot of time. :) Thanks!
