Android application for Recording/Retrieving Information using a MySQL Database

I would like an application built using Java/HTML and MySQL (for recording/retrieving/viewing information from a database; you will have to populate this database with some data yourself). The application will be able to access the native camera API (ideally using the Cordova plugin) and take a photo that will be saved into a database alongside other information. A simple JQuery Mobile theme should be used for design and other components such as sliders, checkboxes, drop-downs should be used where necessary. Application should be written as a single html file and contain code comments as well as references to any external libraries used so I could maintain/expand on the application in the future if need be. Further information (such as app designs) will be provided to successful candidate. Preferred turn over time is around 4-5 days if possible. Since my budget is restricted to roughly £60-70 bidders in that range are far more likely to be considered. Trustworthy candidates only please.
