Basic android form app and web panel.

Hi, We need android application and really basic web panel (need to make php&mysql). Please read carefully. All detail explained in here. Its really basic to make, we just need login with qr code, after login user can see his old appointment, and can resend again someofone, also can send new form. Also all of one need to see on web panel. Details : In Android app; User will login with QR code (defined before in web panel, when try to login it will confirm from web service) After login; In the header User will see his name, surname and profile pic taken from webservice (php mysql) In the content area user will see + (ADD button) When user click this button, redirected to new activity and in this activity user will see kind of form. In this forms - can select items(taken from webserver, its like code 001-text,002-text,003-text) and can click start button. - When user select items after he can click the start button. But After user start button it needs to confirm from master QR Code, this confirmation made from "Master QR Code". After this master QR code reading from android, it will post this form with user details and forms content to web service again. - Master user defined in the web panel. In the content area user will see his old form appointment in a listview. - There is also "button" in every item. User can click this button, but some of appointment cant click again. It means we need "statu" item in json, if statu = E, user can send again(before master qr approve this) with same content, if status is not = E user cant do anything. After login every transaction need to confirm with master qr code. Master qr code can confirm form post and repost. Thats all.
