ASP/ASP.net page to Write Survey Results to Access DB

I have an MS Access database to store employee evaluations. I am in the proccess of putting this online. I have an ASP.net (2.0) app written, but I am having a problem creating one major input screen. I have about 60 associates that get rated on a scale of 1-5. These employees are rated by a "Tech" I need 2 pages created that list all employees and all five ratings in a crosstab type view. One page would be for the tech to initally save/enter the info. The second page would be an edit page that would look the same, but give the tech the option to edit/change the info and also show new associates that need evaluations. The ratings categories would be radio buttons with only one value per employee per tech. The tech would go to the page choose their name from a pull down. I already have the Access Db created and will share with winning bidder. I am attaching a dummy Exel file to give you a better idea of what I am looking for. The results are written to a table that holds the tech/employee (primary key) and the score. This can be written in either ASP or ASP.net, but I do prefer VB if you use .net. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Windows Server 2003 with ASP.net 2.0 and IIS 6. MS Access, SQL 2000, SQL 2005 Express Windows Sharepoint Services 2003 installed.
