Build email form for wiki

Create a simple email form template on wiki. Allow user (content owner) to customize with different form fields and set the recipient of the form for submission. Form should be similar to Google Docs Form (doesn't need to be complicated!). Specify Question Title, Question Type (Text, Paragraph Text, Multiple choice, Checkboxes), and whether question is required) Wiki documentation regarding email form: [login to view URL] This can be written using Javascript to do the form validation and use either PHP script or AJAX function to perform the mail action.
