Customize ApPHP Medical Appointment Script

The project is to customize ApPHP Medical Appointment (demo here: [login to view URL]) with the following features: 1. Currently the script have list of countries. I need to allow admin to add list of subcountries (states) for each country, add list of city for each subcountry 2. Currently the script allow to add especialities. I need to be able to add type of services, and for each service add especialities 3. Currently the script allow to browse espacialists by name, especiality and location. I need to be able to select type of service, then select esppeciality for that service, which then display especialists. 4. After display especialists I need to be able to refine results by country, subcountry and city, using drop dow list of country, subcountry and city. 5. I need to allow users to rate especialists (5 star rating), add comment to especialists, recommend especialists, add especialists to favorites.
