BarCode Reader Software with .CSV Import

Hello - I need a software in either PHP or a Windows Based solution which would allow me to do the following :- There is a Bar Code we have which generate as soon as the order gets process in our online store i.e. COD 128-A We have a .CSV file which we import every day and it has the following field like Please also see the attached .zip file which has the true example you can either make this software in PHP using MYSQL and or windows based what ever is easy, in that software you would allow me to either import .csv or that .csv file be stored in our D:\ Drive if you are making windows software and that will be used as soon as we SCAN bar code which will only have CODE 128-A bar code which is also attached in zip file for example purpose, and based on that scan it will allow us to make a packing list, so basically in easy words you will make me a software in either windows or php which will keep our .csv file and in that .csv we have all the entries which will be display, print and make a packing list for our warehouse person to generate shipping label. That will happen as soon as you scan that barcode in CODE 128-A That software will be access based on username/password.
